No Contracts — Month-to-Month
Month-to-Month Medical Marketing. No Contracts. Cancel Anytime.
We don’t lock medical practices into 6-month or 12-month contracts. Every client is month-to-month from day one. If the work isn’t producing results, you leave. If it is, you stay because the numbers justify it — not because a contract forces you to. We believe the agency that needs a contract to retain clients is telling you something about the quality of their work.

The standard agency model in medical marketing is a 6-month or 12-month contract with an early termination penalty. The stated justification is always some variation of “marketing takes time to produce results” — and that’s true, it does. But the contract isn’t there to protect the timeline. It’s there to protect the agency’s revenue during the months when the work isn’t producing results and the client would otherwise leave.
Here’s what the contract model actually protects: the agency’s ability to underperform for 3–6 months without consequence. The agency that needs a 12-month contract to retain a medical practice is telling you that their retention rate without the contract would be lower — and that means their results aren’t strong enough to keep clients voluntarily. The contract substitutes for performance.
The month-to-month model flips the incentive structure. The agency has to produce visible progress every single month because the client can leave at any time. There’s no “we’re still in the ramp-up phase” excuse at month 5 when the contract prevents the client from acting on their frustration. The agency either delivers or loses the account. That’s the accountability structure that produces better work.
How Month-to-Month Actually Works at Tandem
No minimum commitment. You start, we work. If at any point you want to stop, you give 30 days’ notice and we wrap up cleanly. No penalties, no early termination fees, no guilt trip calls from an account manager trying to save the contract.
You own everything. Your Google Ads account, your Meta Business Manager, your analytics, your content, your landing pages, your call tracking numbers, your data. We work inside your accounts, not agency-owned accounts. If you leave, you keep everything we built. Nothing is held hostage.
Full access and transparency from day one. You have real-time access to every campaign, every report, every dollar spent. No black-box reporting where the agency controls the dashboard and sends you a PDF once a month.
Biweekly status calls, monthly reporting. Not quarterly check-ins where problems compound for three months before anyone addresses them. Biweekly calls to review performance, discuss optimization decisions, and align on priorities. Monthly written reports with CPL, conversion, and ROI data by campaign.
30 days to prove value, every month. We treat every month as a retention event. The question we ask internally every month is: “Would this client re-sign today if they had to make the decision fresh?” If the answer isn’t a clear yes, we have a problem to fix — not a contract to hide behind.

What We Do for Medical Practices
Tandem is a specialty medical marketing agency. We manage Google Ads, Meta advertising, SEO, and the conversion infrastructure that turns clicks into booked patients — for medical practices specifically.
Google Ads management. Service-segmented campaign architecture, hyperlocal geo-targeting, call tracking with dynamic number insertion, ZIP-level bid adjustments, negative keyword management, weekly optimization, and monthly CPL reporting. $1,250/month or 12% of ad spend.
Meta & Instagram advertising. Geo-fenced prospecting and retargeting campaigns, UGC and before/after creative direction, CAPI implementation, audience building, and monthly reporting. $750/month additive.
Medical SEO. Treatment-page architecture, provider authority pages, FAQPage and medical schema, local SEO and Maps Pack optimization, AI search citation optimization, and monthly organic reporting. $750–$1,750/month by tier.
Conversion infrastructure. Landing page audit, booking widget integration, call tracking setup, Enhanced Conversions, GA4 event configuration. Included in management.
Medical Specialties We Serve
We work across medical specialties because the paid advertising and SEO mechanics transfer — but the clinical context, patient decision cycles, competitive dynamics, and compliance requirements don’t. We build specialty-specific campaigns, not generic healthcare templates.
Fertility clinics and IVF. Long research cycles, international patients, treatment-specific landing pages, HIPAA-compliant creative. Medical spas. Service-segmented campaigns, hyperlocal geo, UGC Meta creative, membership conversion. Urgent care. Walk-in volume, geo-fenced radius, seasonal demand, multi-location. Orthodontics. Dual-funnel (adult cosmetic + parent-driven), Invisalign competition, specialty entity signals. Surrogacy agencies. Two-audience campaigns, compliance navigation, state-level legal variation. Dental, dermatology, plastic surgery, OB/GYN, orthopedics, pain management, behavioral health, ophthalmology, chiropractic, spine.
What “Month-to-Month” Doesn’t Mean
It doesn’t mean we cut corners on onboarding. The first 30 days include full account audit, campaign architecture build, conversion tracking setup, landing page recommendations, and competitive analysis — the same scope a contract agency delivers, without the lock-in.
It doesn’t mean we don’t invest in long-term strategy. SEO compounds over 6–18 months. We build for the long term and communicate the timeline transparently. You stay because the trajectory is visible, not because a contract forces you through the flat months.
It doesn’t mean higher prices. Our pricing is the same whether you stay one month or three years. No setup fees. No onboarding fees. No “discounted rate that resets if you cancel.”
It doesn’t mean we disappear if you leave. Clean handoff documentation, full account access transfer, 30-day transition support. Your campaigns keep running. Your data stays intact.

Red Flags in Medical Marketing Agency Contracts
12-month minimum with early termination penalty. The agency expects a meaningful percentage of clients to want to leave before 12 months. The penalty exists because they know some clients will be unhappy.
Agency-owned ad accounts. If the agency runs campaigns in their own accounts, they own the campaign history, Quality Score, conversion data, and audience lists. If you leave, you start over from zero. This is leverage, not service.
“Setup fee” waived with commitment. A $3,000–$5,000 setup fee that gets waived with a 12-month contract is a termination penalty by another name.
No raw data access. If you can’t see your own Google Ads account and GA4, the agency controls the narrative. Transparency requires raw data access.
Vague deliverables. “Ongoing optimization” without specific campaign counts, content deliverables, reporting cadence, and optimization actions. Vague scopes protect the agency, not the client.
Frequently Asked Questions
Is there really no contract?
Correct. Month-to-month from day one. 30 days’ notice to cancel. No early termination fees. No penalties. No minimums. We have a simple service agreement that defines scope, pricing, and responsibilities — not a lock-in contract.
Why don’t you require a contract if marketing takes time?
Marketing does take time — especially SEO. But the right response is transparent communication about timelines and visible early indicators of progress, not a contract that prevents the client from acting on justified frustration. If we’re executing well and communicating the trajectory, clients stay.
Do I own my ad accounts and data?
Yes. We work inside your Google Ads account, your Meta Business Manager, your GA4, your call tracking. Everything is in your name. If you leave, you keep everything.
What happens in the first 30 days?
Full account audit, campaign architecture build or rebuild, conversion tracking setup, call tracking, landing page recommendations, and first campaign launches. Google Ads typically produces first leads within 14–30 days.
How much does it cost?
Google Ads: $1,250/month or 12% of ad spend. Meta: $750/month additive. SEO: $750–$1,750/month by tier. No setup fees. No onboarding fees. Same price whether you stay one month or five years.
What specialties do you serve?
Fertility clinics, medical spas, urgent care, orthodontics, surrogacy agencies, dental, dermatology, plastic surgery, OB/GYN, orthopedics, pain management, behavioral health, ophthalmology, chiropractic, and spine.
What if I want to cancel?
30 days’ notice. Clean handoff documentation, full account access transfer. No penalties, no pressure. If the work isn’t producing results, you should leave.
How is this different from freelancers?
Infrastructure and depth. We run Google Ads, Meta, SEO, and conversion infrastructure as integrated systems with biweekly calls, monthly reporting, and specialty-specific medical marketing expertise.
Do you work outside California?
Yes. We work with medical practices across the United States. All management is location-independent.
What’s the catch?
There isn’t one. We believe the agency that produces results retains clients without needing a contract. Our retention comes from performance, not legal enforcement.
